You can access the module under Administration > Settings > Signups > Custom Form Fields.
Click on the button “+“.
Enter the name of the field (e.g., age, phone number, etc.).
Input: choose according to the format in which you want to collect the requested information from applicants:
text (single-line) – text on one line.
textarea – longer text allowing multiple lines.
whole number – only whole numbers can be entered.
decimal number – decimal numbers can be entered.
checkbox – you can check or uncheck it.
select – fill in multiple options for users to choose from. You can check Allow selection of multiple options for users.
date – select a date with an option to Allow time selection: requiring both the date and exact time (hh:mm) to be filled.
file – upload/submit a file.
verified address – select an address from the registry (available for CZ, coming soon for other countries).

After clicking "+ Add more options", you can specify the custom field further:
Common for all types
Displayed name: if you want to show a different field name to users than the one you set in the settings.
Help text below the field: a help text to provide extra context to users filling it.
Placeholder text: a hint value that appears in the background of empty fields.
Suffix: useful for units (kg, cm, m, etc.) displayed after the field and its value.
Internal note: only visible to platform admins in the custom form fields module.
Text (single-line)
Minimum and maximum text length: limit entered value by character count.
Text (multi-line)
Minimum and maximum text length: limit entered value by character count.
Number (whole)
Minimum and maximum value: limit the entered value.
Number (decimal)
Minimum and maximum value: limit the entered value.
Step size for the counter.
Selection of values – when checking the option Allow selection of multiple options
Minimum number of selected options: – define the minimum number of options users must choose.
Maximum number of selected options: – define the maximum number of options users can select.
Date
Max. possible date: limit the date selection from above.
Min. possible date: limit the date selection from below.
By pressing the OK button, you save the custom field.
Click on the specific Signup> Signup Steps.
In a specific step, Signups> Add a new field > Select From Library – you will see all pre-prepared fields to choose from.

Edit and Delet: A custom form field can be edited or removed anytime if the user has the permissions. A deleted custom field will disappear from all signups it was assigned to, including all saved values!
